When to Refer a case to the CatCare Program

1. Click here to download the CatCare Referral Form.
2. Click here to download the Word version of the Referral Form.
3. Use the “Tab” key to move between each section, entering the necessary information.
4. Complete both the Injured Worker Information and Workers’ Compensation Insurer Information sections.
5. Save the file to your desktop and email it to catcare@bestdoctors.com or fax it to 1-877-762-8150.
6. An Advanced Practice Nurse will contact you upon receipt of the referral. If you have any questions about completing the sections listed above call 1-866-237-3286.

Note: If you are unable to save the PDF version of this form with your entries completed, please print it out and fax it to 1-877-762-8150.

Legacy Program
